Selective development of muscular force in the rehabilitative context: methods in comparison

 

The study evaluates the efficacy of the mechano-acoustic vibration therapy compared to the isokinetic training in subjects with quadriceps muscle hypotrophy.
Results show an improvement in strength development and in work performing ability with both treatments. However, the vibration treatment provides better results and is the only one to be effective in the contralateral muscle.

 

Abstract

Aim. The mechanical vibrations, when applied to the muscle and the tendon structure, provokes the “tonic glare vibration” stimulus with consequent increase of the muscular contracted force. The adaptation involves the neuromuscular system, specifically the motor centres to obtain a better recruitment of the muscular fibres by the central nervous system.
Method. Twenty feminine subjects (age 31 ± 12 years, height 162 ± 5 cm, body mass 66 ± 4.6 kg) affected by marked quadriceps hypotrophy have been randomly divided into 2 groups (A and B). The right quadriceps muscle was treated in all subjects, with the following modalities: group A: 10 rehabilitative sessions, 5 per week with mechano-acoustic vibration (300 Hz); group B: 10 sessions in alternate days with isokinetic machine Cybex, 90°/s.

Results. Group A: the isokinetic test at 180°/s showed an increase in Pf of about 61% at T1 (p<0.05) and 56% at T2, while the results of the test at 60°/s are a 77% increase at T1 (p<0.05) and a 71% increase at T2 (p<0.05). Group B: the isokinetic test at 180°/s showed an increase in Pf of about 52% at T1 (p<0.05) and 18% at T2, while the results of the test at 60°/s are a 14.1% increase at T1 (p<0.05) and a 7% increase at T2 (p<0.05).

Conclusions. The local vibratory stimulation leads to a significative increase in muscular tone and strength. This stimulation can be effectively used in the rehabilitative therapies.

Sviluppo selettivo della forza muscolare nel contesto riabilitativo: metodi a confronto


Lo studio valuta l'efficacia della terapia vibrazionale meccano-acustica rispetto all'allenamento isocinetico in soggetti con ipotrofia muscolare dei quadricipiti.
I risultati mostrano un miglioramento nello sviluppo della forza e nella capacità di eseguire il lavoro con entrambi i trattamenti. Tuttavia, il trattamento a vibrazione fornisce risultati migliori ed è l'unico ad essere efficace nel muscolo controlaterale.


Astratto
Scopo. Le vibrazioni meccaniche, quando applicate al muscolo e alla struttura del tendine, provocano lo stimolo della "vibrazione del riflesso tonico" con conseguente aumento della forza contratta muscolare. L'adattamento coinvolge il sistema neuromuscolare, in particolare i centri motori per ottenere un migliore reclutamento delle fibre muscolari da parte del sistema nervoso centrale.
Metodo. Venti soggetti femminili (età 31 ± 12 anni, altezza 162 ± 5 cm, massa corporea 66 ± 4,6 kg) affetti da marcata ipotrofia dei quadricipiti sono stati divisi casualmente in 2 gruppi (A e B). Il muscolo quadricipite destro è stato trattato in tutti i soggetti, con le seguenti modalità: gruppo A: 10 sessioni riabilitative, 5 a settimana con vibrazione mecano-acustica (300 Hz); gruppo B: 10 sessioni a giorni alterni con macchina isocinetica Cybex, 90 ° / s.
Risultati. Gruppo A: il test isocinetico a 180 ° / s ha mostrato un aumento del Pf di circa il 61% a T1 (p <0,05) e il 56% a T2, mentre i risultati del test a 60 ° / s sono un aumento del 77% a T1 (p <0,05) e un aumento del 71% a T2 (p <0,05). Gruppo B: il test isocinetico a 180 ° / s ha mostrato un aumento del Pf di circa il 52% a T1 (p <0,05) e il 18% a T2, mentre i risultati del test a 60 ° / s sono un aumento del 14,1% a T1 (p <0,05) e un aumento del 7% a T2 (p <0,05).
Conclusioni. La stimolazione vibratoria locale porta ad un significativo aumento del tono muscolare e della forza. Questa stimolazione può essere efficacemente utilizzata nelle terapie riabilitative.
